Akron Children’s Community Health Needs Assessment (CHNA) Parent & Caregiver Survey

Are you a parent or caregiver of a child under 18? If so, Akron Children’s Hospital wants to hear from you. The Hospital is conducting a survey to identify issues relating to children’s health and well-being in your community.
